What You'll Do

  • Conduct telephonic assessments and evaluations for patients, ensuring comprehensive care management.

  • Utilize your bilingual Spanish skills to communicate effectively with Spanish-speaking patients and families.
  • Document patient interactions and care plans accurately in our electronic health records system.
  • Provide education and resources to patients regarding their health conditions and treatment options.
  • Collaborate with interdisciplinary teams to ensure continuity of care and optimal patient outcomes.
  • Monitor patient progress and adjust care plans as necessary based on clinical evaluations.
  • Maintain compliance with legal and regulatory requirements in case management practices.

Requirements

  • Active RN license in the United States.
  • 2-5 years of experience in case management or telephonic nursing.
  • Fluency in Spanish is required.
  • Strong organizational and communication skills.
  • Experience with electronic health records and case management software.
  • Knowledge of workers' compensation and healthcare regulations.
  • Ability to work independently in a remote setting.
  • Excellent problem-solving skills and attention to detail.
