Care Management Coordinator - Remote Opportunity
About the Role
High Desert Medical Group is seeking a full-time Care Management Coordinator to join our dedicated team. This remote position is perfect for individuals who are passionate about providing exceptional support in healthcare. As a Care Management Coordinator, you will play a vital role in assisting our Care Management Team, including Acute Case Managers and Complex Case Managers, ensuring that our patients receive the best possible care.
What You'll Do
- Demonstrate care, compassion, concern, and courtesy when interacting with internal and external customers.
- Answer phones, transfer calls, and take messages with appropriate telephone etiquette.
- Perform clerical duties such as copying, filing, and preparing mail for distribution.
- Enter new patients into NextGen and distribute faxes to appropriate parties.
- Call physicians' offices for appointment scheduling as needed.
- Complete Excel reports when necessary and perform other duties as assigned.
Requirements
- High school diploma or equivalent; relevant experience in healthcare or administrative roles preferred.
- Strong communication skills and ability to demonstrate empathy in patient interactions.
- Proficiency in Microsoft Office Suite, especially Excel.
- Experience with electronic health records (EHR) systems, preferably NextGen.
- Ability to multitask and manage time effectively in a fast-paced environment.
Nice to Have
- Previous experience in a care management or case management role.
- Knowledge of medical terminology and healthcare processes.
- Familiarity with patient scheduling and appointment management.
