About the Role

We're seeking a dedicated Remote Clinical Liaison to join our innovative healthcare team. In this role, you will leverage your expertise as a Registered Nurse, Paramedic, or Licensed Clinical Social Worker to facilitate patient transitions and enhance care coordination across various healthcare settings. As a Remote Clinical Liaison, you'll play a crucial role in ensuring patients receive the appropriate care and support they need.

What You'll Do

• Act as a primary point of contact for patients, families, and healthcare providers to ensure seamless transitions of care.
• Utilize your clinical knowledge to assess patient needs and develop tailored care plans.
• Collaborate with multidisciplinary teams to optimize patient outcomes and satisfaction.
• Manage referrals and coordinate services to enhance patient care pathways.
• Provide education and support to patients and families regarding healthcare options and resources.

Requirements

• Current licensure as a Registered Nurse (RN), Paramedic, or Licensed Clinical Social Worker (LCSW).
• Minimum of 3 years of clinical experience in a healthcare setting.
• Strong communication and interpersonal skills to build relationships with patients and healthcare teams.
• Experience in patient management and care coordination.
• Proficiency in electronic medical records (EMR) systems.
