Remote Cardiometabolic Registered Nurse - Patient-Centered Care

About the Role

We are looking for a Remote Cardiometabolic Registered Nurse to join our innovative team at 9amHealth. As a Remote Cardiometabolic Registered Nurse, you will play a crucial role in delivering patient-centered health coaching and chronic disease management. This position offers a unique opportunity to work remotely while making a significant impact on the lives of individuals managing chronic conditions such as obesity, diabetes, high blood pressure, and high cholesterol.

What You'll Do

• Deliver patient-centric health coaching by integrating medical needs with patient preferences.
• Triage patient-reported symptoms and escalate to Providers when appropriate.
• Provide education on diabetes, hypertension, hyperlipidemia, and weight management.
• Coordinate care between patients, providers, and clinical teams to ensure seamless communication.
• Create consults aligning with internal protocols for Providers to enhance patient care.
• Document patient care accurately to meet internal standards and regulatory requirements.

Requirements

• Bachelor of Science in Nursing (BSN) required.
• Active Registered Nurse license in good standing.
• Certified Diabetes Care and Education Specialist (CDCES) or willingness to pursue certification.
• 3+ years of direct patient care experience in an ambulatory or telehealth setting.
• Strong empathy and communication skills to effectively engage with patients.
• Adaptable and comfortable working independently in a remote environment.
• Proficiency with digital tools for care delivery.
• Flexibility for rotational weekend or holiday coverage.

Nice to Have

• Experience with chronic disease management programs.
• Familiarity with telehealth platforms and electronic health records.
• Additional certifications in nursing specialties.
