About the Role

We are seeking a dedicated and qualified Licensed Social Worker to join our team in a remote capacity. Working as a Licensed Social Worker means being part of something special: a team that is passionate about making an impact on our patients’ lives every day. Unlike the typical hospital setting, this role offers you the unique opportunity to walk alongside patients on their road to recovery from various conditions. As a Licensed Social Worker, you'll care for and help patients achieve their goals and regain independence, forming significant relationships and celebrating their successes along the way.

What You'll Do

  • Plan, develop, organize, implement, evaluate, supervise, and direct social services programs and activities.
  • Ensure an ongoing quality assurance program, developing and implementing appropriate plans to correct deficiencies.
  • Implement social service interventions that achieve treatment goals, addressing resident needs and linking social supports to enhance quality of life.
  • Consult and coordinate social services activities with administration, medical and nursing services, and other related departments.
  • Participate in community planning and the development and implementation of social care plans and resident assessments.
  • Provide information to residents and their families concerning available support agencies and financial assistance programs.
  • Maintain incident and accident reports related to behavior management and elopement.
  • Participate in pre-admission planning and placement of residents in the facility.

Requirements

  • Minimum of a Bachelor’s Degree in Social Work.
  • Must meet Federal and State required Certification/Licensure for intended facility.
  • Previous experience in healthcare preferred.
  • Ability to deal tactfully and positively with personnel, residents, family members, and the general public.
  • Must have patience, tact, enthusiasm, and a positive attitude toward the elderly and disabled.
  • Must be able to speak, read, write, and understand English.
