Intake Coordinator - Remote Home Health Services

About the Role

We are looking for an Intake Coordinator - Home Health to join our team remotely. In this role, you will be the first point of contact for new patient referrals, ensuring a smooth and efficient intake process while providing high-quality home health care services.

What You'll Do

• Accept incoming calls for new referrals for clinical services provided to home care patients.
• Collect all necessary data for patient admissions, including demographic data, medical history, medication, and insurance information.
• Verify insurance and payer sources using DDE or specific systems.
• Ensure referral acceptance and communication within 15 minutes of receiving a referral.
• Notify the clinical designee responsible for scheduling and authorizations.
• Liaise between field, medical, office, and referral source staff.
• Obtain authorization from insurance companies and ensure physician licensure compliance.
• Conduct follow-ups on current active patient hospitalizations.
• Maintain a non-admit log and attend in-services and meetings.

Requirements

• High school diploma or GED equivalent.
• Prior clinical home health experience preferred.
• Knowledge of Medicare, TennCare, and accreditation standards.
• Proficient in Microsoft Office Suite, including Outlook, Word, and Excel.
• Familiarity with medical terminology.
• Excellent verbal, written, organizational, and time-management skills.
• Strong initiative and attention to detail.
• Demonstrated confidentiality and professionalism.

Nice to Have

• Experience with electronic health records (EHR) systems.
• Certification in healthcare administration or related field.
• Ability to work independently and as part of a team.
