Care Coordination Specialist - Hybrid Remote Position
About the Role
We are seeking a dedicated Care Coordination Specialist to join our team at Alive Hospice. This hybrid remote position allows you to work from home 2-3 days a week while also requiring presence in our Nashville, TN office. As a Care Coordination Specialist, you will play a crucial role in supporting our providers and clinical staff by managing essential administrative and care coordination tasks. Your ability to prioritize and manage a diverse workload in a fast-paced healthcare environment will be key to your success in this role.
What You'll Do
- Process new referrals and field initial interest in services in a personable and professional manner.
- Handle prior authorizations for medications, diagnostics, and durable medical equipment accurately and promptly.
- Coordinate referrals to specialists, ensuring required documentation and insurance approvals are obtained.
- Manage laboratory and diagnostic results, ensuring they are reviewed, documented, and communicated according to provider instructions.
- Perform order follow-up, tracking outstanding labs, imaging, and referrals while closing loops with patients and providers.
- Maintain accurate updates in the electronic medical record (EMR) system.
- Prioritize daily workload to address urgent authorizations, time-sensitive results, and referral deadlines effectively.
Requirements
- High school diploma or equivalent required.
- A minimum of 2 years of experience in healthcare administrative roles.
- Familiarity with EMR systems and medical terminology.
- Experience working with Medicare and commercial insurance plans.
- Preferred: Experience in hospice and/or palliative care settings.
Nice to Have
- Experience in outpatient healthcare settings.
