About the Role
We are looking for a dedicated Arizona Long Term Care ALTCS Case Manager to join our team at Banner Health. This remote position allows you to manage and coordinate care for members in the AHCCCS population, ensuring their needs are met through comprehensive assessments and service planning. As an Arizona Long Term Care ALTCS Case Manager, you will play a crucial role in enhancing the quality of life for our members while working from the comfort of your home.
What You'll Do
- Manage care for Arizona long term care members, ensuring they receive the necessary support and resources.
- Conduct thorough assessments and develop individualized service plans tailored to each member's needs.
- Coordinate with various healthcare providers to ensure seamless care delivery.
- Travel to members' homes for evaluations, providing personalized attention and support.
- Collaborate with interdisciplinary healthcare teams to create comprehensive care strategies.
- Assist members in achieving their personal goals and advocate for their needs.
- Work Monday to Friday, from 8 AM to 5 PM, ensuring consistent availability for your members.
- Participate in community functions to enhance member engagement and support.
Requirements
- Bachelor's degree in social work is required.
- Two years of experience working with elderly or disabled individuals.
- Strong knowledge of healthcare systems and regulations.
- Excellent communication skills to effectively interact with members and healthcare teams.
- Ability to develop and implement service plans that cater to individual needs.
- Experience in patient advocacy is highly valued.
- Bilingual skills are preferred for some assignments.
- A valid driver's license is required for travel to members' homes.
- Understanding of AHCCCS regulations is essential.
