We are seeking a dedicated and experienced Telephonic Case Manager (RN) - Bilingual Spanish - Remote to join our team. What You'll Do

• Conduct telephonic assessments and evaluations for patients to determine their healthcare needs.
• Coordinate care plans and facilitate communication between patients, healthcare providers, and insurance companies.
• Utilize your clinical knowledge to provide guidance and support to patients navigating their healthcare options.
• Document patient interactions and maintain accurate medical records in compliance with regulations.
• Assist patients in understanding their treatment options and benefits, ensuring they are informed and engaged in their care.

Requirements

• Active Registered Nurse (RN) license in the United States.
• Fluency in Spanish is required to effectively communicate with patients.
• 2-5 years of experience in case management or a related field.
• Strong organizational and communication skills to manage multiple cases effectively.
• Experience with telephonic nursing or remote patient management is preferred.

Nice to Have

• Certification in case management (CCM or ACM) is a plus.
• Experience in workers' compensation or utilization review.
• Familiarity with electronic medical records (EMR) systems.
