Reactivation Specialist - Remote Opportunity in Elderly Care
About the Role
We are seeking a dedicated Reactivation Specialist to join our team in providing exceptional care for elderly residents at WZC Breugheldal. This remote position offers an opportunity to make a meaningful impact in the lives of our residents, focusing on their psychological well-being, engagement, and participation.
What You'll Do
- Work collaboratively within a multidisciplinary team to provide individualized support and group activities.
- Encourage independence and social interaction among residents.
- Contribute actively to person-centered care and approaches.
- Promote dementia-friendly care principles, ensuring autonomy and normalization for residents.
- Support residents with Activities of Daily Living (ADL), including meal assistance.
- Observe resident behavior and participate in care planning discussions.
- Initiate and signal needs while collaborating closely with colleagues, families, and external partners.
- Participate in monthly team meetings and contribute to dementia-friendly initiatives.
Requirements
- A relevant bachelor’s or master’s degree in fields such as occupational therapy, speech therapy, or social work.
- Strong organizational skills and the ability to work independently.
- Enthusiasm and motivation that inspire both residents and colleagues.
- Experience in elderly care or a related field is preferred.
- Ability to adapt and implement person-centered care strategies.
Nice to Have
- Experience with dementia care principles.
- Familiarity with the TUBBE care model.
- Previous experience in a multidisciplinary team setting.
