Requirements

• Active and unrestricted LPN/LVN license in Massachusetts.
• Strong understanding of clinical decision-making processes.
• Experience in primary care or chronic disease management is preferred.
• Proficiency with EMR/EHR systems to manage patient records.
• Excellent verbal and written communication skills for effective patient interaction.
• Strong organizational skills and attention to detail in documentation.
• Collaborative mindset in a fast-paced, tech-forward environment.
