Case Manager - Remote Advocate for Community Support
About the Role
We are seeking a dedicated Case Manager to join our team remotely, advocating for underserved individuals in your community. As a Case Manager, you will play a vital role in providing support and resources to those in need, ensuring they receive the care and assistance they deserve.
What You'll Do
- Establish positive working relationships through a solution-based approach with individuals served and their support teams.
- Develop and monitor Person-Centered Plans (PCPs) that identify outcomes important to the individuals.
- Advocate on behalf of individuals, providing education and resource information to empower them toward self-determination.
- Conduct assessments and gather necessary documentation for eligibility of services.
- Research and coordinate resources to achieve outcomes specified in individuals' PCPs.
Requirements
- Bachelor's degree in a human services field or a non-human services field with two years of relevant experience.
- Experience working with individuals with developmental disabilities is preferred.
- Strong communication skills and ability to build relationships with diverse groups.
- Ability to adapt to changing situations and maintain a customer-focused approach.
- Familiarity with HIPAA regulations and confidentiality practices.
Nice to Have
- Knowledge of Maryland Department of Health’s Targeted Case Management.
- Experience in advocacy and resource coordination.
- Skills in person-centered planning and documentation.
