Senior Social Worker - Remote Opportunity
About the Role
We are seeking a Senior Social Worker to join our team at Educational Alliance, offering a remote opportunity to make a significant impact on the lives of older adults. As a Senior Social Worker, you will maintain a caseload of clients, providing essential support and services that enhance their well-being and independence.
What You'll Do
- Maintain a caseload of older adult clients, conducting comprehensive assessments of needs and providing case management.
- Formulate ongoing care plans based on clients' challenges, strengths, and needs, and make referrals to external agencies.
- Connect clients to applicable resources and intervention strategies, monitoring and evaluating the effectiveness of interventions.
- Collaborate with on-site nurses and interdisciplinary teams to ensure consistent service delivery.
- Oversee the Senior Companions Friendly Visiting Program and facilitate group activities.
- Participate in biweekly case conferences and attend required meetings.
- Maintain up-to-date case records and program statistics in the documentation database.
- Assist with organizing NORC and OAS events and provide program coverage as needed.
Requirements
- Must hold an LMSW and have a minimum of 1 year of experience working with older adults or in social services.
- Knowledge of resources and entitlements, as well as age-related cognitive and mental impairments, is required.
- Strong attention to detail and the ability to work under pressure while coordinating a comprehensive array of services.
- Ability to take initiative, work independently, and prioritize tasks effectively.
- Excellent verbal and written communication skills, with a collaborative approach to teamwork.
- Proficiency in MS Office (Outlook, Word, Excel) is required.
