About the Role
We are seeking a dedicated and experienced Remote Nurse Practitioner to join our team at Ennoble Care. In this role, you will provide essential primary care and palliative care to homebound patients in the Mercer County, NJ region. As a Remote Nurse Practitioner, you will apply your medical expertise to ensure that our patients receive the highest quality of care in the comfort of their homes.
What You'll Do
- Provide high-quality care to primary and palliative patients in and around Mercer County, NJ.
- Assess patients' health by performing physical examinations and updating medical histories.
- Develop and implement personalized care plans for patients with chronic diseases.
- Effectively communicate with office staff and clinical operations across various internal platforms.
- Attend quarterly medical staff meetings to discuss patient care and operational improvements.
- Foster trust and consult with patients, families, and caregivers during visits.
- Order diagnostic testing as medically necessary.
- Collaborate with physicians to ensure comprehensive patient care.
Requirements
- Active Nurse Practitioner license in New Jersey.
- Minimum of 2 years of experience in a clinical setting, preferably in home care or palliative care.
- Strong communication and interpersonal skills.
- Ability to work independently and manage a diverse patient panel.
- Proficient in using electronic health records (EHR) systems.
- Commitment to providing compassionate, patient-centered care.
