Care Coordinator - Remote Position

About the Role

We are seeking a dedicated Care Coordinator to join our team in a fully remote capacity. As a Care Coordinator, you will play a vital role in supporting members with chronic and complex health needs. What You'll Do

• Conduct outbound and inbound telephonic outreach to members, caregivers, and providers.
• Support nurses and care managers by gathering member updates and documenting interactions.
• Assist with scheduling and coordination of medical appointments and services.
• Perform follow-up calls after hospital discharges and major medical events.
• Identify barriers to care and escalate concerns to appropriate staff.
• Educate members on health plan benefits and preventive care resources.
• Maintain accurate documentation of member interactions in accordance with HIPAA regulations.
• Collaborate with an interdisciplinary care team for coordinated support.

Requirements

• Active certification as CNA, LNA, CHHA, CMA, or Medical Assistant.
• Minimum of 1 year of experience in healthcare settings.
• Strong verbal communication skills with an empathetic phone presence.
• Comfortable with electronic documentation systems and Microsoft Office.
• Able to work independently in a remote environment.

Nice to Have

• Experience with care management or case management.
• Knowledge of healthcare regulations and compliance.
• Familiarity with telehealth services.
