Housing Case Manager - Remote Supportive Services
About the Role
We are seeking a dedicated Housing Case Manager (HCM) to join our team remotely. As a Housing Case Manager, you will play a crucial role in supporting Plymouth residents by working collaboratively with them to enhance their housing stability. What You'll Do
- Develop and maintain Housing Service Plans in collaboration with residents to address their transition and long-term housing stability needs.
- Provide care coordination and supportive services, ensuring residents access necessary resources and appointments.
- Advocate and collaborate with community providers to ensure continuity of care for residents.
- Respond to crises and emergencies, employing de-escalation techniques and focusing on harm reduction principles.
- Maintain accurate documentation of resident interactions and program compliance, ensuring confidentiality and data protection.
Requirements
- BA/BS degree in a human service field or equivalent experience.
- Minimum of two years of experience working with homeless and low-income individuals.
- Valid Washington State Driver License and insurable driving record.
- Excellent written and verbal communication skills.
- Proficient in Microsoft Office applications and ability to work with customized databases.
